REBATE INSTRUCTIONS

Step 1. Check to See if your Utility is Participating

Over 90 North Western Utility Suppliers Participate in the Energy Smart Rebate Program.

Step 2. Buy and Install LED Lights

Call us today at 518-406-8065 for a quote. Let us know how many lights you are looking to retrofit.

Step 3. Mail in Rebate Forms with Invoice

Fill out the following two forms:

*Panoptics LED Light Tubes are a High Power Retrofit

T-12 to LED High Power Retrofit Rebate $28.00/ linear ft (5ft LED Tube $140 / 4ft LED Tube $112)

T-8 to LED High Power Retrofit Rebate $15.00/ linear ft (5ft LED Tube $75 / 4ft LED Tube $60)

3C. Include a copy of your Panoptics Invoice

3D. Mail your Panoptics Invoice, Rebate Worksheet & Rebate Application to:

Step 4. Receive Your Rebate Check

Rebate checks are received by mail approximately 4 weeks after submitting your invoice and the two forms above.
